Motor Vehicle Accidents
If you have been in a motor vehicle accident, insurance companies require you to seek treatment within 10 days of the collision. Momentum Chiropractic serves as an initial assessment and therapy provider for minor injuries due to motor vehicle collisions. You do not require a referral from a medical physician to process documentation and receive treatment at our clinic. Assistance with filling out the required forms (AB1 and AB2 forms) and coordination of insurance claims will be provided upon presentation to the clinic. The clinic will bill the insurance company directly for your treatment.
